Built in the 16th century in the 'head of the left nave', the large organ case was moved to the counter-facade of the church in the 19th century to accommodate the new instrument by Luigi Lingiardi of Pavia. The balustrade is adorned with three panels with carved trophies of musical instruments.

The instruments represented are:

Left panel: portative harp, cittern, double oboe, horn

Central panel: violas, cornett, trumpets, trombone, flutes (?)

Right panel: sistra, tambouine, oboes.
